Please download and fill out the Application Forms(Click on the button below to download the forms, which include the credit application, PA Tax Exemption form, NJ Tax Exemption form, and our Pre-Payment Policies).
Once the forms are complete, please email them to credit@michells.com. You may also fax the completed forms to (610) 265-1142.
Once the forms have been submitted, our Credit Department will review them for approval.
Once your application has been approved, you will be issued a login name and password.
You're now ready to sign in, view our product availability, and place orders!
